Job summary

Family of Kidz is seeking a Physical Therapist to join our school-based team across Long Island. You will design and implement individual and group therapies for children, assess progress, and collaborate with school staff and families to support goals.

Responsibilities include evaluations, case management communication, scheduling, data collection on IEP goals, and participation in CSE or annual reviews. Licensure in NY is required; bilingual skills are a plus.

Qualifications

• Graduate from an accredited university with a degree in Physical Therapy.
• Current New York State Physical Therapy License.
• Bilingual is a plus.

Responsibilities

• Design and implement creative individual and/or group therapeutic interventions for all children on your caseload.
• Administer and write up physical therapy evaluations.
• Ongoing communication with lead supervisors & coordinators, specifically related to case management.
• Maintain professionalism with families, school teams, and other professionals.
• Maintain accurate and timely attendance and billing logs.
• Create and maintain a schedule with assigned individual families/school personnel.
• Collect data on each IEP goal and reports on progress quarterly.
• Provide input and attend/teleconference initial CSE meetings and/or annual reviews.
