Job summary

LOGAN Community Resources is seeking a licensed Physical Therapist to partner with Early Learning programs serving birth to age 5 in St. Joseph County. The role involves evaluating gross motor development, creating treatment plans, and communicating progress to families and staff.

You will work across home and community settings, provide parent training, and help coordinate equipment and adaptive supports. This is a full-time First Steps/PT role focused on child and family outcomes in Indiana.

Qualifications

• Registered and licensed Physical Therapist in Indiana.
• Experience with pediatric populations (0-5) preferred.
• Ability to lift up to 50 pounds during sessions.

Responsibilities

• Perform evaluations and develop treatment plans for gross motor development.
• Collaborate with families and staff to implement home and community programming.
• Maintain progress notes and discharge summaries per payor guidelines.
• Communicate results and recommendations to families, teachers, and partners.
• Attend case conferences and participate in service planning meetings.
