Job summary

Ascend Rehab Services Inc. seeks a California-licensed Physical Therapist to provide Early Intervention home-based services to infants and toddlers (0–3) across Sonoma County.

This is a full-time role that requires traveling between patient homes with flexible scheduling. Qualifications

• Active California Physical Therapy License required.
• Pediatric or Early Intervention experience preferred (training available).
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide Early Intervention home visits to infants and toddlers birth–3 across Sonoma County.
• Travel between home visit locations with mileage reimbursement.
• Support families and children to maximize development through family-centered EI services.
• Flexible scheduling within full-time hours.
