Job summary

Responsibilities

• Develop home exercise programs with caregivers.
• Coordinate care with other providers in the patient’s medical team.
• Complete required patient visits weekly as directed.
• Maintain patient charts, progress notes, and logs timely.
• Uphold agency policies, ethics, and applicable laws.
• Meet with agency staff to align on expectations and standards.
• Utilize standard technology (laptop, email, browser) for duties.
