Job summary

Therapy 2000 seeks a licensed Physical Therapist to perform comprehensive pediatric home-health PT under the supervision of a Supervising Physical Therapist. You will develop home exercise programs with caregivers, coordinate care with the patient’s medical team, and complete required visits and charting.

A Texas license is required, with a flexible, reliable approach to changing priorities and travel between patient visits.

Qualifications

• Current/active license to practice Physical Therapy in Texas.
• Reliability and flexibility to shift resources and priorities as needed.
• Ability to use common workplace technology (laptop, email, web browser).

Responsibilities

• Perform comprehensive Physical Therapy treatments under Supervising Physical Therapist goals and treatment plans.
• Develop home exercise programs in partnership with patients’ caregivers.
• Collaborate with other providers in the patient’s medical neighborhood to coordinate care.
• Complete required patient visits weekly and update charts, progress summaries, and logs.
• Meet agency expectations and maintain high-quality patient care per policies and regulations.
