Job summary

Therapy 2000 is hiring a pediatric physical therapist to provide home-based services across Texas communities. You will conduct comprehensive PT evaluations, develop and implement treatment plans, and create home exercise programs in partnership with families.

Qualifications

• 3+ years as a pediatric Physical Therapist.
• Experience in home-health setting.
• Specialty Certifications (example: PCS).
• Bilingual (English/Spanish).

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine therapy needs.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with patients’ caregivers.
• Coordinate care with other providers and disciplines.
• Meet weekly visit targets and complete charting and documentation.
