Qualifications

• 3+ years as a pediatric Physical Therapist.
• Experience in a home-health setting.
• Specialty Certifications (e.g., PCS).
• Bilingual (English/Spanish) is required or preferred.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with patients and caregivers.
• Collaborate with other providers to coordinate care.
• Meet minimum weekly patient visits as required by the agency.
• Maintain charts, progress notes, call logs, and related paperwork.
• Engage with agency personnel to align with expectations and standards.
• Ensure compliance with agency policies, ethics, and laws for home-health pediatric PT.
