Job summary

KidsCare Home Health is seeking a Pediatric Physical Therapist to evaluate and treat children in their homes. The role emphasizes patient and caregiver guidance to restore and enhance function with a focus on home programming and discharge planning.

Qualifications

• Master's Degree in Physical Therapy required.
• Current state-issued professional license.
• Valid state driver license.
• CPR certified.
• Pediatric clinical experience preferred.
• Home health experience preferred.

Responsibilities

• Examine, evaluate, and administer remedial treatments for pediatric patients.
• Provide clinical treatment to patients including screenings, evaluations, direct treatment, and family/caregiver training and counseling regarding status, progress, goals, home programming and discharge planning.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with the patient and family regarding progress, home programs and therapeutic process.
• Refer patients and family members to appropriate services and resources.
• Consult with physician on patient progress and maintain records.
• Maintain supplies and equipment.
• Participate in Quality Assurance and Performance Improvement (QAPI).
• Attend Continuing Education activities and expand scope of service.
• Maintain adequate caseload with appropriate productivity.
• Collaborate with team and mentor students/new staff.
• Comply with company policies and other duties as assigned.
