Responsibilities

• Perform comprehensive physical therapy evaluations using standardized tests to determine patient needs.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with caregivers.
• Collaborate with other providers to coordinate patient care.
• Meet minimum weekly patient visit requirements per agency policy.
• Maintain complete and timely chart updates, progress summaries, and logs.
• Ensure compliance with agency policies, procedures, and applicable laws.
