THERAPY 2000 in Texas seeks a licensed Pediatric Physical Therapist to join a mission-driven home-based care team. You will conduct comprehensive evaluations, develop treatment plans, and collaborate with caregivers and other providers.

Qualifications
- 3+ years as a pediatric Physical Therapist
- Experience in a home-health setting
- Specialty Certifications (example: PCS)
- Bilingual (English/Spanish)
Responsabilités
- Perform comprehensive Physical Therapy evaluations using standardized tests to determine patients’ needs
- Develop and implement treatment plans aligned with physician orders
- Develop home exercise programs in partnership with patients’ caregivers
- Collaborate and coordinate care between providers and disciplines
- Complete minimum patient visits weekly as required
- Meet deadlines for chart updates, progress summaries, call logs, and paperwork
- Adhere to agency policies, procedures, and applicable laws for home-health pediatric PT
