Job summary

Advocate Health is seeking a Pediatric Physical Therapist to provide direct therapy to pediatric patients in a high-demand setting. The role requires a Licensed Physical Therapist with the ability to evaluate, plan, implement and document care across a pediatric caseload, including holiday coverage, and to supervise assistants as required.

Qualifications

• Bachelor's degree in Physical Therapy program required.
• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Provides direct therapy services to patients seven days a week, including holidays.
• Evaluates patients, selects and administers assessment techniques, and develops a care plan with modalities, procedures, exercise programs and education.
• Gathers and interprets data from medical records, physician reports, patient and family, and documents progress and outcomes.
• Documents evaluations, tests, progress notes, discharge notes and charges.
• Communicates evaluation and progress to physicians, referral sources and other clinical staff; coordinates schedules and discharge planning.
• Provides supervision of PTAs and Rehab Tech/Aide as required for licensure.
