Job summary

Home of the Innocents is seeking a PRN Occupational Therapist for Kosair for Kids Complex Care Center, a pediatric skilled nursing facility in Louisville, KY. You will evaluate residents, implement evidence-based plans, and document progress while collaborating with nursing, PT, speech, and the broader care team.

You will supervise COTAs, contribute to care plans, and build meaningful relationships with children and families in a fast-paced 24/7 environment.

Qualifications

• Master's or Doctoral degree in Occupational Therapy or 10 years of OT experience.
• KY Occupational Therapist license (board-certified) in good standing.
• National Child Passenger Safety Certification or ability to obtain within first year of employment.
• Current CPR/BLS certification (employer-provided if not current).
• Strong interpersonal and motivational skills to build trauma-informed relationships.
• Proficiency with EHR documentation and clinical workflows.

Responsibilities

• Conduct comprehensive OT evaluations and develop evidence-based treatment plans.
• Document progress notes, certifications, recertifications, MDS assessments, Care Plans, and discharge summaries in the EMR.
• Collaborate with Nursing, Physical Therapy, Speech Therapy, and care team to optimize outcomes.
• Supervise COTAs in accordance with licensure requirements and support resident-centered goals.
• Contribute to care planning and communicate progress with guardians.
