Qualifications

• Licensed Occupational Therapist (OT) in IL (IDFPR).

Responsibilities

• Designs an appropriate plan of care integrating goals, treatment, outcomes, and discharge plan.
• Educates patients, families, caregivers, students, and staff using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes.
• Performs examination, selecting reliable and valid examination methods relevant to the chief complaint, results of screening, and history.
• Screens patients to determine the need for further therapy services and uses support personnel as appropriate.
• Documents evaluations and plans of care per department policy including goals and patient response to treatments.
• Complies with annual education for DNV Hip and Knee Replacement Program requirements.
• Evaluates impairments related to upper extremity function and establishes a plan of care based on deficits and goals.
• Completes objective testing for ROM, strength, and daily functional tasks; documents in therapy EMR.
• Communicates with referring provider regarding patient progress and recommendations.
