• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Completes evaluations and plans of care with goals and modifications based on patient response.
• Documents evaluations and plans of care according to department policy including goals and progress notes.
• Communicates with referring provider on patient progress and treatment recommendations.
