Qualifications

• Certification: BLS within 30 days (AHA).
• Licensed Occupational Therapist (OT) – Illinois IDFPR.
• Bachelor’s Degree: Occupational Therapy.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance from bed, chair, or floor.
• Adheres to safe and proper body mechanics to minimize injury risk.
• Designs an individualized plan of care with goals, treatment, outcomes, and discharge plan.
• Adjusts care based on patient response and current evidence.
• Educates patients, families, caregivers, students and other health-care providers.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examinations using reliable, valid methods relevant to the chief complaint and history.
• Screens patients to determine the need for further therapy services.
• Utilizes support personnel in line with legal and ethical guidelines.
• Monitors care delivered by support staff and provides feedback.
• Documents evaluations and plans of care per policy, including goals and modifications.
• Completes annual education as assigned for DNV Hip and Knee Replacement requirements.
• Evaluates upper-extremity function impairments and plans care accordingly.
• Completes objective testing for ROM, strength, and daily tasks.
• Documents daily progress in therapy EMR.
• Administers ultrasound and other modalities as appropriate.
• Communicates with referring providers on progress and recommendations.

Education

Bachelor's Degree: Occupational Therapy

Licensed Occupational Therapist (OT)
