Job summary

Homeside Rehab is seeking a compassionate Occupational Therapist or OTA to join our home health team. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, enjoying schedule flexibility and a supportive team environment.

Responsibilities include assessing, planning and implementing OT interventions to improve motor skills and independence, with documentation to regulatory standards.

Qualifications

• Active NYS Occupational Therapy License (OT or COTA) is required.
• 1 year of experience in home health, outpatient, or SNF settings preferred.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.

Responsibilities

• Evaluate patients and create individualized treatment plans based on assessments and goals.
• Provide evidence-based OT interventions to improve motor skills and functional outcomes.
• Maintain accurate documentation per regulatory requirements.
• Collaborate with the clinical team for coordinated care.
• Communicate progress and plans clearly with staff, patients and caregivers.
