Job summary

Homeside Rehab is seeking a compassionate Occupational Therapist or OTA to join our home health team. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, enjoying schedule flexibility and a supportive team environment.

Responsibilities include assessing, planning and implementing OT interventions to improve motor skills and independence, with documentation to regulatory standards.

Qualifications

• Active NYS Occupational Therapy License (OT or COTA) is required.
• 1 year of experience in home health, outpatient, or SNF settings preferred.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.

Job Summary

Homeside Rehab is seeking a compassionate and skilled Occupational Therapist or OTA to join our home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based OT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Qualifications

• Active NYS Occupational Therapy License (OT or COTA) — required
• Basic computer proficiency for electronic documentation
• Preferred: 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings
• Strong clinical assessment and interpersonal communication skills
• Ability to work independently in a home health environment
• Valid NYS driver's license and reliable transportation for home visits
• Pediatric experience is preferred
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings
• Pediatric OT experience

Job Details

• Job Type: Full-time, Part-time, Per Diem
• Location: Buffalo, NY 14215
• Salary: $65 - $100 per hour
• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
