Job summary

Healthier Mississippi Collaborative seeks an experienced RN to assess and develop nursing care plans, coordinate patient admissions and records, and counsel patients and families. You will liaise with case managers, home health, and utilization review to ensure seamless care across settings.

Responsibilities

• Provides patient care by performing problem-focused assessments, planning, implementing and evaluating treatments and interventions.
• Coordinates delivery of care within the practice setting and across health care settings.
• Refers patients to needed services as ordered by the provider.
• Fosters interdisciplinary collaboration to improve quality of care and outcomes.
• Communicates with patients, caregivers, and other health care professionals about patient care.
• Ensures access to safe, effective, timely and equitable care.
• Mentors new staff and participates in orientation and education.
• Maintains patient confidentiality and reports concerns to supervisor.
• Supports a safe and ergonomic environment for patients and staff.
• Adheres to infection control and waste disposal policies.
