Job summary

Navian Hawaii is seeking a Pediatric Nurse Practitioner (ACNP) to deliver compassionate hospice and palliative care for pediatric patients in the field across Oahu. You will collaborate with pediatric physicians, care teams, families, and community partners to optimize symptom management and patient outcomes.

This role requires Hawai‘i APN licensure, DEA/NED certification, and eligibility for NPI enrollment, with 2+ years of pediatric care experience; hospice or palliative care is preferred.

Qualifications

• Hawai‘i RN license required and APN license held in Hawai‘i.
• DEA/State NED certification with full prescriptive authority.
• Eligible for NPI Medicare enrollment.
• Minimum 2 years of direct pediatric patient care experience; hospice/palliative care preferred.
• HPNA certification preferred.

Responsibilities

• Provide hospice/palliative care to pediatric patients and families in collaboration with the care team.
• Consult with primary physicians and Medical Director to develop care plans.
• Prescribe or recommend medications and treatment as appropriate.
• Document findings and formulate care plans based on patient condition.
• Participate in interdisciplinary group meetings and on-call coverage as needed.
• Maintain accurate, up-to-date patient records and support education for staff.

Pediatric Nurse Practitioner (Full Time)

The Pediatric Advanced Practice Registered Nurse (Acute Care) is responsible for working in collaboration with the pediatric patient, primary care physician, medical director, clinical nursing leadership and other pediatric care team members in an active practice to deliver hospice and palliative care and related health care services to patients and families.

Primary Responsibilities

• Available as a consultant to patients, families and staff regarding end-of-life issues and to collaborate on symptom management, improve the patient and family experience of care, and improve the efficiency and effectiveness of resource use
• Provides direct patient care (or assists others to provide such care) to hospice patients and their families in collaboration with the patient’s primary care physician, the hospice physician, hospice nursing staff and volunteers, and other members of the care team including Social Work and Chaplain Services
  • Care is directed at meeting the physical, psychological, social and spiritual needs of the patient and family
• Prescribes or recommends drugs or other forms of treatment
• Records physical findings, and formulates plan and prognosis, based on patient's condition
• Consults with the Medical Director and palliative care physician(s) as needed
• Consults primary physician of services provided and collaborates with other physicians to prepare comprehensive patient care plan
• Monitors and maintains progress of patients and reports progress to Medical Director
• Has the ability to work collaboratively with pediatric hospice employees and volunteers as part of an Interdisciplinary Group
• Participation in Interdisciplinary Group Meetings
• Provides on-call coverage during the extended hours and weekends as needed
• Provides training and continuing education to staff in collaboration with the education department
• Assists in development of clinical practice guidelines/standards/policies/procedures in support of quality palliative care
• Identifies needs within different stakeholder organizations and recommends the development of systems within Navian Hawaii to meet those needs
• Completes required face-to-face visits as assigned
• Providing clinical assessment results to the hospice medical director after making face-to-face visits with patients, as assigned
• Participating in the recertification process and collaborating with the Interdisciplinary Team to promote coordination of patient care
• Maintaining accurate, up-to-date records
• Other related duties as assigned by the Medical Director

Knowledge and Skills Requirements

• Demonstrates sound decision-making in the care of patients and families through use of the nursing process
• Work independently to plan and provide patient care
• Provide holistic nursing care to patients
• Identify psychosocial needs of caregivers and provide basic care to meet those needs
• Establish and maintain effective working relationships with staff, clients, family members, the public, and representatives of other agencies and organizations
• Communicate effectively with all levels of individuals
• Acute care or ICU experience highly preferred

Experience and Education Requirements

• Valid Registered Nurse License in the State of Hawai’i
• Valid Advanced Practice Nurse License in the State of Hawai'i
• Valid DEA/State NED Certification and full prescriptive authority
• Eligible to obtain NPI Medicare enrollment
• 2 years of experience of direct pediatric patient care required
  • Hospice or Palliative experience preferred
• HPNA Certification preferred

Working Conditions

• Indoors in patient homes or Hospice Hawai’i homes, nursing, and acute care facilities; may include outdoors in all types of weather

Work Location

This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle

Hours

• Generally, Monday through Friday 8am-4:30pm or 4 days a week from 8am-6pm
  • Position may require weekend, night, or holiday availability
• Rotating night and weekend coverage
• Requires one week per month on-call availability

$108,000-$140,000 Annually
