Job summary

Primecare Home Care Services, Inc. seeks a compassionate Pediatric Skilled Nurse to deliver in-home nursing care for children with complex medical needs in Georgia. The role involves direct patient care, condition monitoring, and family education within the Georgia Pediatric Program (GAPP).

You will work with physicians, therapists, and case managers to ensure comprehensive care, while upholding state regulations and agency policies.

Qualifications

• Licensure as LPN or RN with a valid Georgia license.
• BLS certification required.
• Minimum 1 year pediatric home care experience.
• Experience with medically complex patients (feeding tubes, tracheostomies, ventilators) desirable.

Responsibilities

• Provide one-on-one skilled nursing care to pediatric clients in the home setting.
• Manage and maintain medical equipment, including feeding tubes, tracheostomies, ventilators, and seizure logs.
• Administer medications, respiratory treatments, and other prescribed therapies.
• Monitor and assess patient conditions, responding promptly to changes in status.
• Document care provided and communicate effectively with healthcare team members.
• Educate and support families in caring for children with complex medical needs.
• Collaborate with physicians, therapists, and case managers to ensure comprehensive care.
• Maintain compliance with all state regulations, agency policies, and professional nursing standards.
