Job summary

MercyFirst in Syosset, NY is seeking a Nurse Care Coordinator (LPN or RN) to provide best-practice nursing care to our child and family clients. Under the Nursing Supervisor, you will work with birth families, foster parents, and the treatment team.

The role is part-time and onsite, with day hours 8am–4pm, Monday–Friday. Current NY license, CPR, child abuse and infection control training, and a valid driver’s license are required.

Qualifications

• Current LPN or RN license in New York State.
• Current CPR certification.
• Completion of Child Abuse and Infection Control training.
• Valid driver's license with satisfactory driving record.

Responsibilities

• Conduct physical health assessments for infants, children, and adolescents.
• Take vital signs, heights, weights; document observations.
• Provide immunizations and document same.
• Administer medications and reconcile meds as ordered.
• Execute medical orders and obtain lab specimens per physician guidance.
• Ensure medical documentation is completed within required timeframes.
• Maintain professional boundaries and a safe medical environment.
