Qualifications include NY LPN license, CPR, and Child Abuse/Infection Control training; minimum 1 year pediatric nursing experience. The role emphasizes care coordination, medical record keeping, and collaboration with providers.

Qualifications

• Current New York State LPN license and CPR certification.
• Completion of required courses in Child Abuse and Infection Control.
• Minimum 1 year of pediatric nursing experience.

Responsibilities

• Coordinate care with the medical home staff and treatment team.
• Collaborate with the Medicaid Managed Care Liaison to address health care needs.
• Coordinate scheduling of primary, dental, mental health and specialty care; obtain records.
• Act as liaison between providers for health services to children in care.
• Document client care in patient and department records; audit records for compliance.
• Communicate with clients, court and multidisciplinary team about health information.
• Participate in case conferences as needed.
• Support caregivers around medical/dental/mental health needs of children.
• Ensure compliance with regulations for medications and care in care settings.
• Liaise with hospitals during hospitalizations and attend related conferences.
