Job summary

Infinite Medical P.C. is seeking an on-site Nursing Care Coordinator (RN or LPN) in Brooklyn, NY to review and verify chart documentation, ensuring CMS compliance and continuity of care for our residents in skilled nursing and long-term care facilities.

You will work closely with HR, Operations, and Clinical teams, maintain proficiency across EMR platforms, and participate in mandatory meetings. The role offers a daytime, full-time schedule with comprehensive benefits and a competitive hourly

Qualifications

• Valid RN or LPN licensure required.
• Experience in geriatric care and long-term care settings.
• Detail-oriented with strong documentation review skills.
• On-site schedule: Monday to Friday, 9:00 AM – 5:30 PM; ability to adapt to occasional extended hours.

Responsibilities

• Review patient records and RVLs for CMS compliance.
• Identify documentation discrepancies and propose corrections.
• Coordinate with HR, Operations, and Clinical teams to resolve inquiries.
• Maintain proficiency across EHR platforms (PCC, MatrixCare, SigmaCare) and Google Workspace.
• Participate in mandatory meetings and cross-train in care management areas.
