MercyFirst is seeking a Part-Time Nurse Care Coordinator (LPN/RN) based onsite in Syosset, NY. The role reports to the Nursing Supervisor within Integrated Health and involves providing best-practice nursing care to infants, children, and teens, collaborating with birth families and treatment teams.

Day shifts are Monday to Friday, 8am–4pm. Must hold NY LPN/RN license, CPR, and required child abuse/infection control courses, plus a valid driver’s license.

Qualifications

• Licensed practical nurse (LPN) or registered nurse (RN) in New York State.
• Current CPR certification.
• Completion of required courses in Child Abuse and Infection Control.
• Must have a valid driver’s license with satisfactory driving record.

Responsibilities

• Conduct physical health assessments for infants, children and adolescents.
• Routine screening for substance abuse, developmental, mental health checks.
• Manage chronic conditions (asthma, diabetes) and document observations.
• Take vital signs, heights, weights and document findings.
• Provide immunizations and document administration.
• Provide first aid and emergency care as needed.
• Execute medical orders as directed by physician or nurse practitioner.
• Obtain lab specimens as required and administer hearing/vision tests.
• Provide health education related to sexual reproductive health.
• Medication administration and reconciliation.
• Ensure documentation for all case actions within established timeframes.
• Maintain professional boundaries and a safe medical environment.
• Accept medication deliveries and store safely.

MercyFirst is a not-for-profit human and social service agency that has been serving children and families in need since we were founded by the Sisters of Mercy/Hermanas de las Misericordia in 1894. Today our agency continues to address the emotional and physical needs of children and families in Brooklyn, Queens, Staten Island and across Long Island through innovative treatments and life-changing interventions. We provide community-based prevention and family foster care services, group homes in the community for struggling children and families within the child welfare and juvenile justice systems, and short-term residential services for unaccompanied migrant children. PROGRAM BACKGROUND:

To meet the needs of the ever-changing foster care population. Our children have often experienced significant levels of trauma, poverty and violence. As they grow and mature, their individual needs change. Our assessments, health counseling, and selected treatments change to meet their new needs. Our children's physical and mental health as well as their strengths must be supported so they can reach their full potential.

POSITION SUMMARY:

Under the supervision of the Nursing Supervisor, the Nurse Care Coordinator will be responsible for providing best practice nursing care to our clients. Our clients are a mix of infants, toddlers, school age children and teens and are male and female. Nurses will work with birth families, foster parents, and other members of each child’s treatment team.

Day shifts Monday - Friday. 8am - 4pm

REQUIRED QUALIFICATIONS:

Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) in New York State, current CPR certification and completion of required courses in Child Abuse and Infection Control. Must have and maintain a valid Driver’s License with a satisfactory driving record.

RESPONSIBILITIES:

• Conduct physical health assessments for infants, children, and adolescents including HIV Risk Assessments, General First Aid, Triage Activities for "sick" calls.
• Routine Screening for Substance Abuse, Developmental Health, Mental health, Physical Body Checks for Signs of Abuse
• Routine management and training regarding chronic conditions (Asthma, Diabetes)
• Take vital signs, heights, and weights; document observations and activities
• Provide immunizations and document same
• Provide first aid and emergency care as needed
• Execute medical orders (tests, tasks, hospitalizations, etc.) as ordered by physician/nurse practitioner/psychiatrist
• Obtain certain lab specimens
• Administer hearing and vision tests
• Training and Health Education related to Sexual Reproductive Health
• Medication Administration and Management Reconciliation
• Ensure required documentation for all case actions (medical notes) are completed within established timeframes.
• Maintain appropriate professional boundaries with clients and staff.
• Maintain a safe and secure medical environment for clients
• Accept medication deliveries, check for accuracy, store safely

Pay Rate:

LPN: $35/hr. Part-time 20 hours/week.
