Qualifications

• Education: Master's degree in Social Work (MSW).
• Experience: Two years of progressively responsible and directly related work experience.
• Licensure/Certification: Valid license as a Licensed Clinical Social Worker (LCSW) preferred.

Responsabilités

• Collaborates with health care teams to treat and plan for social and medical needs of patients by consulting with providers and making referrals.
• Develops and implements patient management plans, including coordination of referrals and documentation of progress notes.
• Provides supportive care to help patients and families cope with illness, educate on diagnosis, treatment options and life adjustments.
• Maintains records and prepares written and oral reports documenting case activity.
• Participates in monitoring and evaluating quality and appropriateness of patient care.
• Performs other duties as assigned.
