Job summary

TriHealth Pediatrics is seeking a Licensed Independent Social Worker to partner with pediatric providers, schools, and community resources to advocate for vulnerable children in Cincinnati.

You will conduct psychosocial assessments, provide crisis intervention, coordinate referrals to community and behavioral health resources, and support care teams to address social drivers of health and improve outcomes.

Qualifications

• Must hold a Bachelor’s Degree in Social Work, Addictions, Counseling.
• 3-4 years of experience.
• Licensed Clinical Social Worker (LCSW) required.

Responsibilities

• Conduct psychosocial assessments and identify barriers affecting child and family health.
• Provide crisis intervention, advocacy, and brief supportive counseling.
• Coordinate referrals to community, educational, behavioral health, and social service resources.
• Collaborate with providers and interdisciplinary teams to support high-risk patients and families.
• Support value-based care, population health, and other initiatives.
• Maintain timely documentation and track outcomes of interventions and referrals.
