Job summary

Community Hospice & Health Services in Modesto, CA is seeking a Pediatric Associate Medical Director to oversee clinical care for pediatric hospice patients, ensuring quality, cost effectiveness, and regulatory compliance.

The role requires MD or DO, California license, DEA, and board certification in hospice/palliative medicine or related fields, plus ability to work with an interdisciplinary team and travel to patients' homes.

Qualifications

• Must be a physician (M.D. or D.O.) licensed in California.
• Board Certification in Hospice and Palliative Medicine, Family Practice, or Internal Medicine preferred.
• Current California license and DEA registration required.
• Valid California Driver’s License and automobile insurance.

Responsibilities

• Provide general management of medical care for pediatric hospice patients and oversee the interdisciplinary team.
• Determine and certify patient eligibility for hospice care with attending physicians.
• Serve as medical resource with education and consultation for teams.
• Manage care plans and ensure services align with the plan of care and patient needs.
• Consult with primary care/attending physician on pain and symptom control and issue prescriptions as needed.
• Participate in committees/projects to improve hospice services and be available for on-call.
• Visit patients in homes, hospitals, SNFs, residential facilities, and Hospice House as required.
• Comply with all CHHS policies including HIPAA, safety, and infection control.
