Job summary

TriHealth is seeking an experienced LPN to join our Medical Home team in a primary care setting in Cincinnati. The 32-hour workweek includes 4 days with rotating Saturdays and offers benefits eligibility as a full-time consideration.

The LPN will perform pre-visit planning, clinical care, and care coordination with physicians and care teams. Strong EMR skills, BLS, and a focus on patient satisfaction are essential.

Qualifications

• Graduate of an approved healthcare program (Required).
• Licensed Practical Nurse (LPN) license required.
• 3–4 years clinical healthcare experience (Required).
• Proficient in EMR and medical office workflows.
• Basic Life Support (BLS) certification required upon hire.
• Strong communication and teamwork skills.

Responsibilities

• Coordinate primary care rooming processes, relevant procedures, immunizations, venipuncture, point-of-care testing, and retinal scans as needed.
• Provide pre-visit planning and manage patient messages via electronic portals.
• Assist in transitions of care and maintain clear communication with the physician care plan.
• Support wellness and chronic disease management with patient education and coaching.
