Affirmed Home Care in Netcong, NJ is hiring a Per Diem Licensed Practical Nurse (LPN) to provide one-on-one pediatric nursing in the child’s home. This role focuses on compassionate clinical care for a 4-year-old with CP and complex devices, with flexible per diem shifts.

Qualifications

• Active New Jersey LPN license.
• 1 year pediatric experience in home or acute care preferred.
• Current BLS certification.
• Authorized to work in the United States.
• Valid driver's license preferred.
• Physical completed in last 12 months.
• PPD/QuantiFERON (or chest X-ray) in last 12 months/5 years.
• MMR immunization in last 10 years.
• Influenza vaccination or exemption.

Responsibilities

• Provide one-on-one skilled nursing care in the client's home.
• Monitor patient health, comfort, and safety during shifts.
• Administer medications and treatments per physician plan of care.
• Manage tracheostomy/ventilator care, GJ tube feedings, nebulizer, chest vest, cough assist, and suction.
• Maintain accurate and timely clinical documentation.
• Collaborate with family and interdisciplinary team to deliver outstanding care.

We are currently seeking a dedicated Per Diem Licensed Practical Nurse (LPN) for an immediate pediatric case in Netcong, NJ. If you enjoy building meaningful relationships with families and providing exceptional clinical care, we'd love to have you on our team.

Patient Information

• Patient: 4-year-old girl
• Diagnosis: Cerebral Palsy, Seizure Disorder, Tracheostomy/Ventilator (T/V), GJ Tube, Nebulizer, Chest Vest, Cough Assist, and Suction

Schedule

• Monday, Wednesday & Thursday: 7:00 AM - 7:00 PM
• Tuesday & Friday: 7:00 AM - 11:00 PM

Pay

• $40.00 per hour

Responsibilities

• Provide one-on-one skilled nursing care in the client's home
• Monitor the patient's health, comfort, and safety throughout each shift
• Administer medications and treatments according to the physician's plan of care
• Manage tracheostomy/ventilator care, GJ tube feedings, nebulizer treatments, chest vest therapy, cough assist, and suctioning
• Maintain accurate and timely clinical documentation
• Partner with the family and interdisciplinary team to provide outstanding patient care

Qualifications

• Active New Jersey LPN license
• Preferred: At least 1 year of pediatric experience in home care or acute care
• Current BLS certification
• Authorized to work in the United States
• Valid driver's license preferred
• Physical completed within the past 12 months
• Current PPD or QuantiFERON within the past 12 months (or chest X-ray completed within the past 5 years)
• Proof of MMR immunization within the past 10 years
• Proof of influenza vaccination or an approved medical exemption
