Job summary

Qualifications

• RN licensure in California (required)
• Minimum two years nursing experience (required)
• Pediatric experience and case management/utilization management experience desirable (preferred)
• Bilingual skills are a plus (preferred)

Responsibilities

• Assess, plan, implement, coordinate, monitor, and evaluate options and services to meet patients' health needs across the continuum of care.
• Coordinate services for a specified group of patients ensuring appropriate setting and resource utilization.
• Collaborate with health care team to achieve desired patient outcomes.
• Work within the Nursing Practice Act and Valley Children's Healthcare policies
