Job summary

Central California Alliance for Health is seeking a Registered Nurse for a temporary pediatric case management role. The position involves developing care plans, educating families and providers, and supporting cross‑department coordination in a hybrid work environment.

The ideal candidate will hold a CA RN license, have an Associate's degree in Nursing, and at least five years of patient care experience with a minimum one year in case management, preferably in acute or outpatient pediatric

Qualifications

• Current unrestricted RN license issued by the State of California.
• Associate's degree in Nursing and a minimum of five years of patient care experience, including at least one year in case management.

Responsibilities

• Develops and manages an individualized comprehensive plan of care for members in the Case Management Program.
• Educates and assists members, families, providers, external agencies and internal departments about the Case Management Program.
• Participates in Quality Improvement studies to evaluate program effectiveness and outcomes.
