Job summary

Pediatric Specialty Care in Point Pleasant, PA seeks a Program Specialist to coordinate services for medically fragile children. You will develop individualized plans, gather interdisciplinary input, and ensure compliant, child-centered care journeys.

The role emphasizes collaboration with clinical, behavioral, and direct care teams, and involves leading meetings, staff training, and ongoing progress documentation within PA 3800/6400 and DHS guidelines.

Qualifications

• Bachelor’s or Associate degree with IDD experience required.
• Proven ability to coordinate interdisciplinary care and develop ISP/IHP plans.
• Must be 21+ with valid driver’s license; CPR/First Aid certification preferred.

Responsibilities

• Develop, manage, and update ISP/IHP and 6-month reviews.
• Coordinate interdisciplinary input for person-centered care planning.
• Lead meetings in compliance with PA 3800/6400 and DHS regulations.
• Train and support staff on individualized plans, behavioral supports, and safety protocols.
• Monitor and document monthly and quarterly progress toward outcomes.
• Maintain communication with families, schools, and external providers.
• Participate in interdisciplinary, QA, and safety meetings.
• Support participation in community-based activities and life experiences.
