Job summary

Duke University Health System in Durham, NC seeks a Pediatric Clinical Pharmacist—ICU Float. This on-site position supports inpatient pediatric care with emphasis on medication therapy management, safety and education for clinicians and families.

Responsibilities include reviewing orders, dispensing, drug information, monitoring therapies, and collaborating with physicians and nurses. Requires North Carolina licensure; PGY2 pediatrics preferred; 40-hour work week with rotating shifts.

Qualifications

• Doctor of Pharmacy Degree OR BS Pharmacy Degree.
• With a BS Pharmacy degree, two years of pharmacy practice experience, or board certification, or completion of an accredited residency.
• With Doctor of Pharmacy degree, no experience is required.
• PGY2 Pediatric Pharmacy residency or PGY1 Pharmacy Residency with pediatric focus plus clinical experience preferred.
• Licensed Pharmacist or eligible to practice pharmacy in North Carolina.

Responsibilities

• Review and monitor patients' medication therapy and orders for safety and efficacy.
• Prepare, label and dispense medications per prescriptions and regulations.
• Maintain dispensing records and oversee medication charge/credit systems.
• Provide drug information and educate caregivers, patients and staff.
• Oversee the medication process and present reports for system improvements.
• Ensure medications are available, procured, stored and maintained.
• Supervise technicians and support staff in dispensing activities.
• Support investigational medications with proper records and documentation.
• Advise physicians on drug therapy and dosing considerations.
• Contribute to education of students and residents.
• Maintain liaison with medical and nursing staff regarding drugs and regulations.
• Conduct medication histories and participate in rounds with multidisciplinary teams.
