Millenia Hospice is seeking an experienced RN for a PRN Case Manager role covering the Dallas–Fort Worth area. You will provide skilled nursing, patient assessments, and symptom management, collaborating with an interdisciplinary team to support patients and families.

The ideal candidate has Texas RN licensure, CPR certification, and 1–2 years of hospice or home health experience, with flexibility for travel and PRN scheduling.

Qualifications

• Current and unrestricted RN license in Texas.
• Minimum 1–2 years of hospice or home health experience preferred.
• CPR certification required.
• Strong clinical assessment and communication skills.
• Ability to travel and work independently.

Responsibilities

• Perform comprehensive patient assessments and create care plans.
• Deliver skilled nursing care and manage symptoms.
• Coordinate with physicians, social workers, and chaplains.
• Educate patients and families on disease progression and care.
• Monitor patient condition and adjust plans as needed.
• Maintain accurate clinical documentation.
• Participate in on-call rotation (PRN).
• Ensure compliance with hospice regulations and standards.

You will work closely with patients, families, and an interdisciplinary team to ensure comfort, dignity, and support throughout the hospice journey.

Key Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing care and symptom management
• Coordinate care with physicians, social workers, chaplains, and other team members
• Educate patients and families on disease progression, medications, and care techniques
• Monitor patient condition and adjust care plans as needed
• Maintain accurate and timely clinical documentation
• Participate in on-call rotation as needed (PRN basis)
• Ensure compliance with hospice regulations and standards

Qualifications:

• Current and unrestricted RN license in the state of Texas
• Minimum 1-2 years of hospice, home health, or related experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively
• Reliable transportation and valid driver's license
• Current CPR certification (required)
