Job summary

LanceSoft is seeking a Hospice RN Case Manager in Denton County, TX. This full-time, permanent role focuses on delivering compassionate end-of-life home care and supporting families throughout the hospice journey.

Ideal candidates will have an active Texas RN license, hospice and case management experience, and proficiency with EMR systems, with WellSky experience preferred. Travel within the territory may be required.

Summary

We are hiring a Hospice RN Case Manager for a full-time, permanent opportunity in Denton County, TX. This position is ideal for compassionate Registered Nurses with hospice experience who are dedicated to providing exceptional end-of-life care to patients in their homes while supporting families throughout the hospice journey.

Location

• Denton County, TX

Salary

$85,000 – $95,000 annually

Additional Compensation

• Mileage Reimbursement: $0.65 per mile
• Cell Phone Reimbursement: $60/month

Schedule

• Full-Time
• Monday–Friday
• On-call rotation required
• Travel throughout assigned territory

Key Responsibilities

• Provide skilled nursing care based on individualized care plans.
• Perform comprehensive patient assessments and monitor changes in condition.
• Administer medications and treatments as prescribed.
• Educate patients and families on hospice philosophy and symptom management.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Maintain accurate and timely EMR documentation.
• Respond to urgent patient needs and provide crisis intervention.
• Ensure compliance with hospice regulations and quality standards.

Requirements

• Active Texas Registered Nurse (RN) License.
• Minimum 1 year of Hospice nursing experience.
• Minimum 1 year of Case Management experience.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication and patient education skills.
• Experience with EMR systems.
• WellSky (Consolo) experience preferred.
