Job summary

First Day Homecare is seeking an experienced LPN to provide skilled nursing care under physician orders for pediatric and adult clients in a home setting. The role requires the LPN license in Michigan, CPR certification, and strong communication with families and care teams.

The nurse reports to the Clinical Supervisor and will document changes, follow care plans, and collaborate with other clinicians to ensure safe, high-quality care.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current license to practice as an LPN in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured where required.

Responsibilities

• Provides professional nursing care as defined by the state Nurse Practice Act.
• Performs care per medical practitioner orders and client care plan.
• Documents changes in orders in the medical record following agency procedures.
• Assists with personal care, monitoring, reporting findings, medication administration, and client teaching.
• Collaborates with the care team and promotes safety for clients and coworkers.
• Follows agency policies and participates in care coordination and meetings.
