Angels of Care Pediatric Home Health in Lubbock, TX is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LVN) for our Pediatric Transitional Nurse Team to support medically fragile children at home. You’ll care for high-acuity patients, train caregivers, and ensure safe continuity of care during transitions.

Qualifikationen

• Active Texas RN or LVN/LPN license required.
• Current CPR certification required.
• Experience with Trach care and Ventilator management.
• Ability to lift and assist pediatric patients.
• High-acuity pediatric experience preferred.

Aufgaben

• Provide skilled pediatric nursing 1:1 care in patients' homes.
• Administer medications, treatments, and therapies; perform assessments.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge planning and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond to emergencies and monitor changes in patient condition.
