Job summary

CentraCare Health is expanding its Pediatric Home Health and Hospice services with a dedicated Case Manager. The role focal point is pediatric care across the service area, building a pediatric footprint, and serving as a clinical resource for Home Health, Palliative Care and Hospice.

You will conduct assessments, coordinate care, and educate families while collaborating with an RN office support team. The position is based in Waite Park with travel to surrounding communities.

Qualifications

• Graduate from an accredited school of nursing required.
• Current Minnesota Registered Nurse license required.
• Current American Heart Association (AHA) BLS Healthcare Provider card required.
• Reliable transportation required.
• Valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage meeting CentraCare’s standards required.
• Minimum of one year of RN experience.

Responsibilities

• Perform comprehensive assessments and care planning.
• Provide wound care and infusion therapy.
• Coordinate patient care across Pediatric Home Health, Palliative Care and Hospice services.
• Educate patients and families about diagnoses, treatment plans, and follow-up.
