Job summary

CareGivers America – NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager for pediatric home health across Lackawanna and Susquehanna counties. This field-based role offers a salary of $70,000–$80,000 and a company vehicle.

The RN will lead care in the home setting, conduct comprehensive assessments, and coordinate with physicians, families, and care teams while traveling within assigned counties.

Qualifications

• Active Pennsylvania RN license (Required).
• 1 year of professional nursing experience preferred.
• Pediatric or home health experience preferred.
• Strong leadership and communication skills.
• Valid driver’s license.
• Current CPR certification.

Responsibilities

• Complete comprehensive patient assessments, including OASIS documentation.
• Develop and oversee individualized pediatric care plans.
• Collaborate with physicians, families, and interdisciplinary care teams.
• Provide supervision and direction to LPNs and Home Health Aides.
• Monitor documentation for compliance and regulatory standards.
• Participate in quality improvement initiatives.
• Maintain a safe and respectful care environment.
