Job summary

CareGivers America – NEPA is seeking a compassionate Registered Nurse to join us as a Per Diem Pediatric RN Case Manager. This role offers flexible scheduling, competitive per-visit pay, mileage reimbursement, and strong clinical support to stay autonomous within a collaborative care team.

You will perform comprehensive pediatric assessments, coordinate care with physicians and families, supervise LPNs and aides, and contribute to quality initiatives while maintaining compliance and patient

Qualifications

Active PA RN License (required); BSN preferred.
2+ years of professional nursing experience, pediatric or home health preferred.
Strong leadership, communication, and organizational skills.
Ability to manage multiple priorities and work independently.
Valid driver's license and CPR certification.

Responsibilities

Complete comprehensive patient assessments, including OASIS data collection, within established timelines.
Collaborate with physicians, healthcare professionals, patients, and families to develop individualized pediatric care plans.
Provide direction and supervision to LPNs and Home Health Aides to ensure quality care.
Monitor clinical documentation for accuracy, timeliness, and regulatory compliance.
Communicate regularly with patients’ families and care teams to coordinate services.
Participate in quality improvement initiatives and staff education.
Promote a safe and respectful care environment for patients and colleagues.
