Hammond LA PCS, part of LHC Group within the Optum family, is hiring a Home and Community Based Services Licensed Practical Nurse to provide direct in-home nursing care per physician orders in the assigned geographic area.

You will document visits in the EMR, coordinate care with the health team, and support patients and families while complying with infection control and regulatory requirements.

LPN licensure in LA, CPR certification, and a reliable vehicle or transportation are required.

Competenze

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Driver's license and access to reliable transportation.
• Ability to work in diverse home environments.
