Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance or access to dependable vehicle.

Responsibilities

• Provides high quality clinical nursing services within the scope of practice and plan of care.
• Documents patient visits thoroughly in EMR per regulatory and payer requirements.
• Participates in the agency’s care coordination, reporting needs and changes in condition to the team.
• Ability to work in varied home settings and with diverse patient populations.
