Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse to provide direct nursing care in patients' homes within the Eunice, LA service area. You will follow the physician's plan of care, document visits in the electronic medical record, and coordinate other patient services under supervision.

You will work under the direct supervision of a Registered Nurse. Requirements include current LA LPN licensure, a valid driver's license with access to reliable transportation, and LA experience.

Qualifications

• Current and unrestricted LPN licensure in state of practice (LA).
• Current driver's license and vehicle insurance, or access to dependable transportation.
• 1+ years of experience as an LPN in LA preferred.

Responsibilities

• Provide direct nursing care as prescribed by physician in patients' homes.
• Document visits and progress notes in electronic medical record.
• Coordinate patient care activities within care plan and with health care team.
• Function in diverse home environments and with varied patient needs.
