Benchmark Human Services is hiring LPNs for HomePointe HealthCare in Indiana to provide in-home nursing care for infants, children, and young adults. The role emphasizes independent work, comprehensive assessments, and plans of care tailored to each client’s needs.

Locations include Spencerville (Mon & Wed, 8-hour shifts) and Hoagland (Sundays). Requires Indiana LPN license, valid DL, and willingness to travel to nearby clients; Trach/vent experience preferred.

Formación

• Active Indiana LPN license.
• Valid driver’s license and reliable transportation.
• Willingness to travel to nearby clients.

Responsabilidades

• Conduct comprehensive client assessments (physical, psychosocial, environmental).
• Develop and follow an individualized Plan of Care and Nursing Care Plan(s).
• Provide skilled nursing interventions and document progress toward outcomes.
• Coordinate client care with the physician and care team.
• Maintain accurate client records and update Care Plans as needed.
