Qualifications

• Valid Indiana Nursing License (LPN) is required.
• Valid Driver's License for client travel.
• Experience with Trach and G-tube preferred.

Responsibilities

• Provides professional nursing care as defined in the nurse practice act.
• Communicates status changes to the RN Clinical Care Manager.
• Follows established Plan of Care and Nursing Care Plans.
• Documents client interactions in the clinical record.
