Job summary

Reserve Care is seeking compassionate pediatric home care nurses in Ohio to provide intermittent skilled nursing services under physician directions. You will assess needs, manage pain and symptoms, and support families in home care settings.

The role requires strong pediatrics experience and the ability to collaborate with physicians and care teams. The position is exempt and involves driving to patient locations across multiple counties in Ohio, with a schedule that includes evening call

Qualifications

• Licensed RN or LPN in the State of Ohio.
• BS or equivalent experience.
• 3-5 years of experience in Pediatrics, or Hospice with the desire to transfer into Pediatrics.
• Experience in pain and symptom management, oncology, public health, mental health.

Responsibilities

• Assess physical, psychosocial and spiritual needs of the patient and family.
• Provide intermittent skilled nursing services to the patient under the patient's physicians' directions.
• Monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems.
• Serve as liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs.
• Provide emotional support to the patient and family.
• Refer and follow up patients and/or family members to other services when appropriate.
• Provide instruction (health teaching) to the patient and family as related to home care.
• Maintain and promptly complete all necessary plan and reports related to home care.
• Participate in care conferences and bereavement meetings.
• Participate in public relations activities when necessary.
• Participate in audit review, quality assurance activities and other meetings as assigned.
• Plan and supervise activities of HHA as related to patient care.

Skills

Ohio RN/LPN license
