Qualifications

  • Must have and maintain an active, unencumbered license (LPN/LVN) in the state of practice.
  • Compact licenses must be transferred to your state of residence within 90 days.
  • Current CPR certification (with hands-on component)—Aveanna can assist in obtaining this after hire if necessary.
  • TB skin test (current within last 12 months).
  • Six months of prior hands-on nursing experience preferred but not required.
  • Reliable transportation is required.

Position Overview

The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, in accordance with the care plan and physicians' orders for each patient, while maintaining confidentiality and professional boundaries.

Essential Job Functions

  1. Deliver and coordinate quality patient care in compliance with physician orders.
  2. Observe and assess patient condition and care needs; report changes appropriately.
  3. Document all activities, assessments, nursing actions, responses, and care coordination promptly.
  4. Participate in, implement, and update the nursing care plan.
  5. Take appropriate nursing actions based on assessments to achieve expected outcomes.
  6. Recognize changes requiring intervention and implement care to prevent or reduce risks.
  7. Comply with all policies, regulations, and standards, including the Nurse Practice Act.
  8. Use infection control measures per OSHA standards to protect staff and patients.
  9. Educate patients and families on disease processes, self-care, and prevention strategies.
  10. Maintain knowledge and skills through ongoing education and evaluations.

Requirements

  • Graduate from an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse.
  • Current CPR certification.
  • Proficiency in clinical assessments, documentation, and compliance with policies.
