The Nemours Foundation is seeking a Social Worker (MSW) to join the Hem/Onc team in Orlando, FL as a full-time member. The role focuses on providing psychosocial support and care coordination for patients and families dealing with kidney disease and dialysis-related challenges.

Responsibilities include comprehensive psychosocial assessments, crisis intervention, and collaboration with an interdisciplinary team to facilitate discharge planning and access to community resources.

Qualifikationen

• Masters Degree in Social Work from a CSWE-accredited program is required.
• Minimum one year of social work experience; 3+ years preferred. Graduate internship may apply.
• Experience with crisis intervention and child/family services is advantageous.

Aufgaben

• Provide psychosocial services to patients admitted to Nemours Children’s Hospital, focusing on psychosocial needs and well-being.
• Assist families in navigating the healthcare system and link to services tailored to needs.
• Conduct psychosocial assessments and provide crisis intervention and case management.
• Collaborate with the healthcare team to ensure effective communication and care coordination.
• Coordinate admissions screenings, psychosocial histories, and documentation for patients and families.
• Assist with domestic violence, child abuse investigations, and community referrals when needed.
