• Psychosocial Assessment- Performs psychosocial assessments taking into consideration family, social, and medical history.
• Resource Support- Collaboration with the health care team in the development of a treatment plan and assisting in the identification of resources.
• Psychosocial Interventions- Provide psychosocial services and counseling to improve emotional coping and adherence to the treatment plan.
• Interdisciplinary Collaboration- Work with the health care team to promote system improvements and coordination of care.
• Collaboration- Partner with patient, family, and care team in safe transition of care and education.
