Qualifications

Responsibilities

• Collects patient/family data to facilitate identification of clinical, educational, psychosocial, and cultural needs and communicates data to health team members.
• Contributes to patient plan of care based on identified needs in collaboration with the health team.
• Plans, prioritizes and organizes patient care tasks to ensure efficient time and resource use.
• Performs routine patient care tasks and medication administration under supervision.
• Reassesses and modifies patient plan of care with the registered nurse.
• Performs other duties as assigned.
